Service Information

This business is part of Portwell Care Ltd.

Cook Street Nursing Care Centre (CSNCC) provides aged care for Resthome and Hospital Level Care. We can provide private care and respite care depending on our bed availability. Always ring & ask as we can be full one day and have beds becoming available the next. CSNCC is an older, purpose built facility, what we lack in 'modern' we more than make up for in staffing levels and care. We work closely with Arohanui Hospice to ensure all palliative care provided is appropriate and individually targeted. Cook Street Health Centre provide our GP services which ensures timely & focused care delivery. Please drop in and see us, ring or email for more info.

Staff

We have a very stable long term staff base who are happy in their roles and are chosen for their interest and capacity to care for the elderly or unwell. Our facility is filled with activity, laughter and kindness.

Aged Care Management

Anna & Wayne purchased Cook St Nursing Care Centre in Nov 2017. Cook St remains family owned and operated, the only one left in Palmy. The benefits of this include the owners are either onsite or available everyday & working with the rest of the team to ensure any issues can be identified and rectified on the spot.
Finah is the Facility Manager and a highly skilled Registered Nurse and Brian Momongan is our Clinical Nurse Leader. We have excellent RN cover on all shifts

Aged Care Activities

There is a comprehensive diversional therapy programme in place at Cook St Nursing Care Centre. Linda is our qualified DT and ensures group and individual needs are catered for. We work in partnership with the Heartland Lions Club where their members and the CSNCC residents are involved in many fun activities - such as Bra Art; Queen's B'day and St Patrick's day celebrations. We have regular contact with our daycare groups, church groups, knitting circle and many outside entertainers booked every month.

Why we have Rest Home Audits

All rest homes and aged residential care facilities are certified and audited to ensure they:

  • provide safe, appropriate care for their residents
  • meet the standards set out in the Health and Disability Services (Safety) Act 2001.

Types of audit and when they happen

Certification / Surveillance Audits
Certification audits happen every 1–4 years. After the audit, rest homes are certified for a set period of time (the exact length depends on how well the rest home performed at the certification audit). Once this time is up, the rest home must be re-audited and its certification renewed.
An unannounced spot audit (also called a surveillance audit) happens around the middle of a rest home’s certification period. The spot audit ensures progress has been made on outstanding areas identified in the earlier certification audit and that standards haven’t slipped.
In addition to audits, rest homes have to report to their DHB on how they are addressing issues found at audit. These improvements are then verified at the next audit event.
Other types of audit
Provisional audits happen when a provider purchases a certified rest home from another provider.
Partial provisional audits happen when a provider wants to add services to their certificate (eg, a rest home adding hospital-level care), when a new rest home is built, or when a provider adds capacity or reconfigures their services (eg, builds a new wing, upgrades rooms). Before 2014, audits for adding capacity or reconfiguring services were referred to as verification audits.
Ministry inspections
Rest homes may have unannounced inspections by the Ministry under the Health and Disability Services (Safety) Act 2001 in the event of a serious complaint.
DHB issues-based audits
DHBs can conduct issues-based audits under the Aged Related Residential Care Contract. For information on these audits please contact the relevant DHB.
